Brick, stone, tile, drywall...

The first one found was the one with the cut wire. That was probably the result of the drywaller cutting out the outlet.

The other one, was the result of the dumb dumb not making sure ONE screw was tight. I have pulled the original permit. The inspector is now in business here - I am giving him the "opportunity" to come out and reinspect each covered outlet and switch to ensure there are no issues. With more than one, he is taking my offer

The screw was loose on the neutral. The house was built in 2002, it's been arching since. I thought the circuit tripped, turns out we were lucky and it just went didn't allow the circuit to complete so everything on it went dead.

This is the crap that burns homes down.

Good Operating Experience, take this for lessons learned and institute an EPRI recommended preventative maintenance plan. Digital Low Resistance checks could of found this issue. Also of note, no station personnel were injured because you were clear of the room when large loads were started.

Insulation is insulation, it doesn't matter if the wiring was made robust for what amperage you run. The insulation will degrade as time wears on. I imagine it may have already been replaced since the 50's if not you will have to replace the wiring eventually.
